In order to hold your child’s practice spot for the upcoming year, follow the steps below and send completed documents and payment to LAC prior to August 31st.
1. Go to www.swimlac.org, print and complete the following forms:
a. LAC Registration Form
b. Code of Conduct Form
c. Medical consent/release form
d. Volunteer obligations agreement2. USA Registration Form (enclosed) – Please review, make changes and sign the bottom.
3. Make your check payable to LAC for $231.00 (USA Swimming and LAC Registrations).
4. Send in all of your documents by August 31st to start swimming in September.
